Hazard: Screws installed improperly can cause the bindings to come loose or pull off the ski board during use, causing the skier to lose control or fall and suffer injuries.

Hazard: During repairs associated with the October 8, 2003 recall, wire nuts were installed improperly, causing the treadmills to overheat and posing a fire hazard to consumers.

Hazard: Surface paints on the recalled helmets contain excessive levels of lead, violating the federal lead paint standard.

Hazard: The swivel, which is attached to a diving mask, could separate while diving. This will result in a sudden loss of the diver's air supply, causing the diver to engage in emergency ascent. This poses a risk of decompression sickness due to rapid ascent or drowning.

Hazard: The chairs can collapse due to faulty rivets, posing a fall hazard to consumers.

Hazard: The steering gear assembly can fail, resulting in an unexpected loss of steering control, posing a risk of injury to the driver and passenger.

Hazard: The trimmer/edger's spool, spool cap and pieces of trimmer string can come loose during use and become airborne projectiles, posing a laceration hazard to the user as well as bystanders.

Hazard: The plastic tubes used to inflate the paddle float could break and deflate, posing a drowning hazard to consumers.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver's air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.

Hazard: Skiers can unintentionally displace a lever at the rear of the binding, which is used for fitting adjustment by ski technicians. If it is fully displaced, it can result in the unexpected release of the binding and possibly cause the user to fall.

Hazard: The recalled spas have a circulating pump and motor assembly, which can overheat and pose a fire hazard.

Hazard: The harnesses could fail during use, resulting in a hunter falling from the tree stand and suffering serious injuries or death.

Hazard: The ladder's plastic steps can be assembled backward on the support brackets. If this happens, the ladder steps can break and the user can fall.

Hazard: These pool heaters can emit excessive carbon monoxide (CO), posing a risk of CO poisoning in the event of a vent leak in an indoor installation.

Hazard: The buckles on the harness's leg loops could fail, posing a fall hazard for climbers.

Hazard: These carabiners have a green button that acts as a safety mechanism to prevent unlocking. The recalled carabiners can unlock unexpectedly without pressing the green button, posing a fall hazard.

Hazard: The selector pin for the weight plates can slip out of its slot if the edges of the pin are worn, allowing the weights to drop suddenly. Falling weight plates can hit consumers using the machines.

Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.

Hazard: The personal transporter can unexpectedly apply reverse torque to the wheels, which can cause a rider to fall. This can occur when the device is tilted back by the Speed Limiter and the rider comes off and then back onto the device within a short period of time.

Hazard: Due to a defect in the sleeve mechanism, the bar can break under the pressure of significant weight, which could injure consumers and bystanders.

Hazard: The harness could be threaded incorrectly. If threaded incorrectly, the webbing will easily slip when loaded. Incorrectly threaded buckles can loosen, which could cause climbers to slip out of the harness and fall.

Hazard: The blade that trims the excess string of these weed trimmers can detach from the plastic shield covering the trimmer head. The blade can be thrown outward, hitting the user or bystander, resulting in a laceration.

Hazard: The triggers on these crossbows could fire when the safety mechanism is moved from "safe" to "fire" position without pulling the trigger. Bystanders could be unintentionally hit by an arrow fired by these bows.

Hazard: Due to a programming defect with the console, the treadmill can unexpectedly increase in speed and elevation when the User Programs 4 or 5 are set for longer than 30 minutes. If this happens, the user could fall and suffer injuries.

Hazard: The cable eye, or round metal tube that holds the cable, could have a fracture, reducing the overall strength of the cable assembly. If the cable assembly were to fail, this would result in a free-fall hazard for the climber.

Hazard: The harnesses could fail during use, resulting in consumers falling from tree stands and suffering serious injuries or death.

Hazard: The resistance pistons on the glider can come off during exercise. If this happens, a person using the glider can fall off the exercise machine.

Hazard: The treadmill can unexpectedly accelerate or decelerate, possibly causing the user to lose control and fall.

Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users.

Hazard: The candle holder is constructed of plastic and could ignite if exposed to flame, posing a fire hazard to consumers.
