Hazard: A seat with loose bolts may not properly restrain an occupant during a crash, increasing the risk of injury.

Hazard: A loss of drive power increases the risk of a crash.

Hazard: Tires that are missing a DOT symbol may be used in inappropriate applications, increasing the risk of a crash.

Hazard: An incorrect label may result in an improperly restrained child, increasing the risk of injury in a crash.

Hazard: An unsecured driver's seat may not adequately restrain the driver during a crash, increasing the risk of injury.

Hazard: Unprotected electrical wiring can overheat and increase the risk of a fire.

Hazard: Battery failure can cause a loss of drive power, increasing the risk of a crash. Additionally, a battery short circuit increases the risk of a fire.

Hazard: Control arm failure can cause a sudden loss of vehicle steering and control, increasing the risk of a crash.

Hazard: Wheel rim insert detachment can create a hazard for other road users, increasing the risk of a crash.

Hazard: Electrical arcing increases the risk of a fire.

Hazard: Wheel lock-up while driving increases the risk of a crash.

Hazard: An unlatched door can open unexpectedly while the vehicle is in motion, increasing the risk of injury.

Hazard: A broken clutch release lever may not disengage, disabling the motorcycle and increasing the risk of a crash.

Hazard: The recalled Taylor Water Technologies reagent bottles sold with test kits contain sulfuric acid, which must be in child-resistant packaging, as required by the Poison Prevention Packaging Act (PPPA). The packaging for the bottle with sulfuric acid is not child resistant, posing a risk of chemical burns to the skin or eyes and internal corrosive burns, if ingested.

Hazard: The recalled crib bumpers violate the federal crib bumper ban, posing a deadly suffocation hazard to infants. Padded crib bumpers are banned by the federal Safe Sleep for Babies Act.

Hazard: The recalled children's bathrobes violate federal flammability regulations for children's sleepwear, posing a risk of burn injuries and death to children.

Hazard: The recalled unassembled Deluxe Pit Kit slot cars violate the mandatory federal regulation for magnets because the magnets used to power the car are loose prior to assembly, posing an ingestion hazard to children. Assembled models do not pose this risk and are therefore not subject to this recall. When high-powered magnets are swallowed, the ingested magnets can attract each other, or another metal object, and become lodged in the digestive system. This can result in perforations, twisting and/or blockage of the intestines, infection, blood poisoning and death.

Hazard: The recalled children's nightgowns, pajama sets and robes violate the federal flammability standards for children's sleepwear, posing a risk of serious burn injuries or death to children.

Hazard: The metal contact in the switch mechanism of the Recon LT avalanche transceiver can corrode and cause the transceiver to malfunction. If this occurs, the device can turn off and make it impossible to locate a victim in an avalanche, which could result in serious injury or death.

Hazard: The stroller fan's housing allows consumers' fingers to contact the fan's blade, posing a laceration injury hazard to small children.

Hazard: Error in the IFU lists an incorrect incubation time on one occasion of 20 minutes rather than the correct incubation time of 60 minutes.

Hazard: Firm became aware of software issues related to most recent firmware update which could result in loss of patient tissue.

Hazard: For certain MR systems, a scan can be resumed following a Power Monitor trip when the "6-minute average SAR" is above the limit indicated on the "SAR Display". The user interface may incorrectly enable a scan to proceed earlier than intended. If resumed by the operator, such a scan can result in potential excessive tissue heating, especially if the patient is in contact with a conductive material or has an implant.

Users have observed smoke-like steam and noted that the light guide at the tip is hot during use / after the endoscope is removed from the patient, and may cause thermal injury to the patient's mucous membranes.

Hazard: During endoscopic procedures using a combination of the video processor EPK-i8020c and the i20c series video endoscope, the observed image can become reddish or dark (1.8%). Users have observed smoke-like steam and noted that the light guide at the tip is hot during use / after the endoscope is removed from the patient, and may cause thermal injury to the patient's mucous membranes.
